Blood Collection
We provide an appointment-led community phlebotomy service.


Community Cardiac Team
This service provides care and support to patients with cardiac conditions, predominantly heart failure, especially following a recent hospital admission, a new diagnosis or recent intervention.


Community Matrons
We provide clinical assessment, patient education and support to enable people to manage their long term conditions and prevent avoidable hospital admissions.


Community Neurological Rehabilitation
The Community Neurological Rehabilitation Team aims to help people regain skills and promote their functional independence following an acute stroke or brain injury and for acute management of long-term neurological conditions.


Community Rehabilitation
The Community Rehabilitation team consists of highly qualified and experienced Physiotherapists and Occupational Therapists.  We visit patients who are unable to to access outpatient services and require therapy at home.


Continence
The service is provided by a specialist team of nurses who are available to all patients with a bladder or bowel issue.  This team provides specialist clinical assessment, individualised treatment programmes and education and support.


Diabetes Nursing
The diabetes nursing service consists of a group of diabetes specialist nurses and diabetes nurse educators who have had extra experience and training in the management of all types of diabetes.


District Nursing
The District Nursing Team provide care to patients who have a short-term nursing need and specialise in addressing the needs of patients in the community.  We are experienced in delivering care in a home setting and from treatment centres at GP surgeries.


Falls Prevention
The Falls Prevention Service offers assessment to people over 65 in their own home if they have fallen or are fearful of falling.


Integrated Urgent Community Response Service (UCR)

Our multi-disciplinary team, led by advanced nurse practitioners provide an urgent response to patients who are unwell at home and at a high risk of an unplanned admission to hospital, if their condition does not stabilise.

Nutrition and Dietetics
Registered dietitians use scientific research on nutrition, health and disease to provide practical advice to help people make appropriate food choices.


Podiatry
Podiatry is a specialist area of health care which provides assessment and treatment of conditions affecting the foot.


Respiratory team

The respiratory team offers home oxygen assessments and support services to patients with respiratory conditions across the district. Our respiratory nurses also deliver the community pulmonary rehab programme for patients in the Skelmersdale area.


Skelmersdale Walk-in Centre
We can help people with busy lifestyles that need access to flexible and convenient health services and may have difficulty accessing a convenient appointment at their registered practice.


Speech and Language Therapy
Adult Speech and Language Therapy Services provide assessment, therapy, advice, information and support for adults and their families or carers, who have difficulties with communication (speech, language or voice) and/or swallowing.


West Lancashire Urgent Care Centre (Ormskirk Walk-in Centre)
West Lancashire Urgent Care Centre at Ormskirk Hospital is open every day of the year including public holidays, for people needing urgent help with minor illnesses or injuries.
