Community Emergency Response
We provide a rapid response service to prevent avoidable hospital admissions as well as short-term rehabilitation. Care will be delivered either in the patient’s own home or in a community bed based facility according to need.

The Community Emergency Response Team is a multi-disciplinary team providing holistic short-term care and rehabilitation as appropriate. The team comprises:
- Nurses
- Occupational Therapists
- Social Workers
- Technical Instructors
- Health Care Assistants
The remit of the team is twofold:
- To prevent unnecessary admissions to hospital and enable patients to remain in their own homes during an acute phase of illness
- To facilitate an early discharge following a stay in hospital
Hospital prevention
You will be provided with care as indicated by your condition.
Home from hospital
On your return home you will be assessed by the Occupational Therapist and Physiotherapist, who will provide a customised programme of rehabilitation for your needs.
Intermediate Care Bed from hospital
You will be admitted to a bed-based facility to promote your recovery with the intention of enabling your return home.
Home from Intermediate Care Bed
If needed, your therapy input will continue on your return home for a limited period of time from the same team.
