Community Matrons

The Community Matrons team are experienced health professionals, who work with people with long term conditions and who have had unplanned admissions to hospital, relating to their long term conditions.

The Community Matrons team can offer a home visit and assessment, to identify any concerns you may have regarding your health and social care needs. The Community Matrons can plan your care, and work closely with your GP and other health and social care professionals, to help you manage your conditions.

We provide clinical assessment, patient education and support to enable people to manage their long term conditions and prevent avoidable hospital admissions.

We cover Skelmersdale, Upholland, Ormskirk, Parbold, Burscough, Tarleton, and Banks in Southport.
